Abstract

The integration of digital solutions in healthcare has transformed patient support and education services, enabling personalized, data-driven, and scalable care models. This paper explores the design, development, and implementation of patient support services and patient educator programs using the Salesforce Health Cloud platform. It examines key features such as patient engagement automation, AI-driven insights, care coordination, and interoperability with electronic health records (EHRs). Additionally, the study highlights best practices for leveraging Salesforce Health Cloud to enhance patient experience, improve adherence to treatment plans, and optimize healthcare provider workflows. Through industry case studies, the paper showcases successful implementations, addressing challenges such as data security, compliance, and user adoption. The study concludes by discussing future trends and strategic recommendations for healthcare organizations looking to innovate patient support and education through Salesforce Health Cloud.
